The US HAEA Angioedema Center at UC San Diego is Here to Help If:

• You or a relative have been diagnosed with angioedema
  
• You or a relative suspect that you may suffer from angioedema
  
• You or a relative would like a second opinion on your angioedema care or diagnosis
  
• Making your first appointment
  
• If you need translation services or interpretations for the hearing impaired, please let us know in advance. We will be happy to accommodate you
  

In Advance Of Your First Appointment We Would Appreciate Receiving The Following Items:

• Information about your current treatment
  
• Medical records including clinic notes, ER notes, hospital summaries, X-rays, lab results, or pathology reports related to your current condition
  
• Written list of medications you are taking, both prescribed and over the counter. Include vitamins, supplements, and herbal remedies
  
• The forms you received from us by email
  

At Your First Appointment Be Prepared To Bring The Following Items:

• Your health insurance card(s), including Medicare or Medi-Cal cards
  
• Any co-payment required by your insurance
  
• A list of questions for your doctor and a pen and paper to take notes
  
• Bottles of medications you are taking, both prescribed and over the counter. Include vitamins, supplements, and herbal remedies
